Caring for neighbor's sick cat

About eight months ago, my next door neighbor asked me if I would feed her cats while they go on vacation this summer. They had about a week before adopted two kitties from the Humane Society. I agreed to do that. One of them is just fine, but the other has been sick almost nonstop. The cat has UTIs, has definite litter box issues, regularly poops behind furniture and doors, pees all over the house to the point where she ruined some carpet, and regularly has diarrhea on the carpet and the wood floors. She has been on various medications since they brought her home. My neighbor talks to me about this all the time, and I am very sympathetic.

As summer approaches, I am thinking about their vacation. I don't want to be responsible for a sick cat, and I don't want to clean up cat messes all over the floors. I know it would be a terrible thing if I took care of the cat and didn't clean it up, because it would definitely ruin their floors and carpet.

I see my neighbor regularly, but she has not mentioned anything about the vacation. I feel like I should approach her about it before they go, so they can make other arrangements. I just don't know how to do this. I don't want this to ruin our neighborhood relationship, because they are good neighbors, as am I.

Can anybody offer me advice on how to handle this rather delicate situation?
