Cat randomly pees outside the litter box

~2 year old fixed female cat. We've had her for about 6 months now, and haven't had any real issues. She's peed outside the litter box three times now, over a span of about as many months. Once in a cardboard box we had laying out, once in the entryway closet, and once under a desk maybe ten feet from her litter box. The first time, she was having some digestive issues and we took her to the vet and found she'd eaten a shoelace. Nothing was found the second time, and the third time happened like 20 minutes ago, but there's no other signs something is wrong. She does not seem to have any issues using the litter box normally, and no issues eating or drinking.

None of the normal reasons seem to fit. Her litter box is clean and lidless and in an as out-of-the-way corner of our apartment as we have, she's the only cat in the house and she can't see any other neighborhood cats (second floor apartment, the only cat that comes around doesn't seem to stray far enough that she'd be able to see it out a window. There haven't been any disruptions to her schedule, but there will be soon as my spouse is going on deployment soon and she's fairly attached to him, so I'd like to get some idea on what to do sooner rather than later.

Any suggestions on what might be causing her to randomly pee outside the litter box, and how I can try to stop it?

Edit: Fourth time in three-ish months. She also peed behind the washing machine one evening. There weren't any medical or apparent stress/environmental issues that time either.
