Cat with a skin condition behaving oddly now

My cousin has 3 cats that he took in as strays (two were kittens literally dumped into the sewer, the other was older and his eyes were frozen shut wandering the streets of Philly). He recently moved in with me and I helped take them all to the vet primarily because of some symptoms the older cat (Chance) was having.

Months before he moved in with me, Chance started scratching his ears, face and neck bad. His ears are nearly bare and he would have these black "balls" (I said they were scabs, cousin said they were mites... vet said they aren't mites) stuck in his fur around his ears and head. He also started scratching his side raw and his good eye (the other eye was damaged before my cousin found him) was oozing.

Took him to the vet and the vet diagnosed him with a bacterial and/or fungal infection in his ears and a potential autoimmune disorder causing the worst of the itching on his side. She gave him antibiotics, antifungal ear drops, eye drops and an oral steroid as well as a cloth cone collar to prevent him from scratching his face apart (which he was none too happy about).

For a few weeks, he seemed to improve markedly. The scratching was less frantic, though he would still rub his ears on anything he could to scratch them a bit. But then this past week, he hasn't come downstairs for anything but food or water. This is weird because he'd kinda been my buddy since they moved in and he'd usually just chill with me on the sofa. Chance kinda bonded with me while the other 2 are afraid of me (don't worry, my cousin's closest with the younger boy and my cousin's GF is closest with the girl) and I kinda considered him mine moreso than the other two. Today, I noticed Chance sleeping in the bathroom and he wouldn't leave when I tried to get him out. He started sneezing and shaking. The black spots on his fur are back and his fur seems a lot more patchy (we've taken the collar off since it's gotten pretty beaten up and he'd been doing better). I'm really worried about him and I don't really know what else I can do to help him. The itchiness must be driving him nuts, but that's not the end of the world. I'm more concerned that there's something worse going on that's making him isolate and behave oddly.
