Cat is VERY sick, roommate doesn't clean her litterbox

Despite my warnings that he wouldn't be able to properly take care of one, my roommate got a cat months ago. She was perfectly healthy. He would only clean her litterbox once a month and it smelled absolutely terrible, so after weeks of me complaining his solution was to get an enclosed lid for her box.

As the months go by, his cat develops a terrible, wet cough. More time passes and he takes her to the vet. She is put on antibiotics but the cough doesn't go away. I do some research and tell him that her litterbox is probably killing her, especially considering it has a lid on it now and is holding ALL of the ammonia and toxic gases that develop.

I keep telling him that he needs to clean it more often, so he cleans it once. Her cough improves slightly within the next few days, but is back and as terrible as ever within 2 weeks. I tell him he needs to regularly clean it or she will die, he says its not the litterbox because he cleaned it and she wasn't better within 2 weeks.

He seriously only cleans it once a month (probably not even once a month). Is this definetly what is making his cat sick? I wonder if I should just start cleaning it, but I feel like I shouldn't, because it is his cat and his responsibility and I told him not to get one. If you think this is what is making her sick, how can I convince my roommate that she will die if he doesn't prevent ammonia from building up in her litterbox?
