Cat no longer using the litter box

I know this is a common problem here, but I feel like I've tried every possible solution with no results. I have three cats who are all around four to five years old and get along well, all fixed and relatively healthy. One, a female, has had issues with constipation and not pooing in her box before (I normally have four, have upped that to six recently), and Miralax and amitriptyline seemed to stop this behavior.

A month ago she began to poo outside the box again. Two and a half weeks ago, she stopped using the litter box completely and has been sweet as pie while peeing on everything since. No preferred spot, room, or surface. The vet gave her a clean bill of health over the weekend after almost a thousand dollars in tests, and I've been using Feliway multicat plug-ins for a week now. She's also back in amitriptyline.

I ended up having to crate her. It's a large crate with tiers, with a large open litter box and Cat Attract litter, and she seems pretty happy in there, but she's still ignoring the litter box completely. I've had to remove all fabric, including a scratching post she started to pee on instead, and a few hours after I did that, she peed on herself.

I don't know what to do anymore. I hate doing this to her, but she won't even consider a box anymore, and I've tried over half a dozen different types of litter. My vet is hesitant to try Prozac because of urine retention, but I'm at the end of my rope. She's sweet and cuddly and adores me, and that behavior hasn't changed, but I just can't figure out what she wants. I absolutely do not want to rehome her, and I'm desperate for solutions I haven't already tried.
