i really need help with my cat

so I've had my cat for about a year and a half now and i got him with the intention of an indoor cat. we have a two story house and we have this routine where we open the balcony on the second floor so he can run around up there and get some fresh air. in the past few days he's discovered that he can safely jump off the balcony into the garden and basically go anywhere. now I'm really stressed because i know it's cruel to keep him in if it's his heart's desire to be out but im too scared to leave him free reign outside. what if he never comes back? what if he gets hurt? how do i make sure he's safe and knows to come home at night? it's all too much for me because this issue just happened so suddenly and we thought he was happy with the balcony. any advice would really be appreciated as im losing sleep over this

side note: he's also been throwing up a lot lately i dont know if it's related
