3 yr old male cat acting very strange

My male cat who we've had for over 2 years now (shelter and adopted at 6 months) has had a few issues with a swollen uretha that we had to take him to the vet 2 times in the past for (he's never had a full or partial blockage). It's been about 2 months since his last visit/meds and he's been peeing fine since.

He's an indoor cat in a small family setting with basically no visitors/no sudden change in environment. The only thing I can think of is introducing him to a new cat tree a few days ago.

He's still been affectionate around us but his tail has been vibrating periodically and he's hiding in areas he usually doesn't hide. He peed on the bed (which he never does), and the stain was huge so I know he doesn't have a blockage.

The weather hasn't changed, and our routines have not been any different since his change in behavior. I'm extremely concerned but want to know if anyone has any other ideas as to why he may be acting like this before I take him to the vet again (he hates the vet and takes a day or two to readjust after his visits)
