How do I know when it’s time?

I have a border collie who just turned 4 years old. She is my whole world. She has a lot of health problems such as very arthritic knees, torn CCLs in both legs, and now her front paws are developing carpal hyperextension (she’s walking on wrists instead of pads) that is due to her not having strength to carry her back end and all her weight is shifted to the front. She also has spondylosis of her lumbar spine. She’s on Gabapentin for the nerve pain, tramadol and now also meloxicam instead of prednisone. She can’t really walk unassisted and I made a sling to support her rear end weight so she can go potty. I do use diapers overnight for accidents which occasionally happen. She is struggling to get from a sitting to a standing position without help and her leg muscles in the back have begun to atrophy. She has a great appetite and drinks water but she is not active and cannot play or walk without help. I carry her everywhere so she is never alone and she sleeps in bed with me every night. She is still affectionate and gets excited to go lay outside. These issues have been happening for almost two years now and prior to this year, she would “bounce back” and start walking again- that’s not happening this time. I’m obviously going to talk to her vet and see what she thinks but I wanted some opinions and or advice before I do so as I can’t even talk about it without crying.
