Litter trained cat peed in cardboard box (4yo male)

I've read that this could be the cause of some bladder issues and am considering taking him to the vet. However, I was also wondering if it's possible that he may have just confused the cardboard box for his litter box. Should I take him to the vet or wait to see if his behavior changes?

Additional info:

  • I recently change his litter from clay to pine pellets but he's been using the new litter without issues for the past 4-5 days.
  • His diet consists of only dry food as he refuses to eat wet food. However, he doesn't seem to have any problems drinking enough water from what I can tell.
  • the box he peed in was in a room adjacent to the room his litter box is in.
