Nail trimming question

My mother and I have waited way too long to trim our dog's nails. He won't let the vet do it, i honestly think it's because it's too painful. We got him seven months ago and haven't trimmed his nails since, he's a doberman. I looked it up on the internet and it says that trimming their nails when theyre too long it's painful. This was a punch to the gut. Im aware this is neglect and i desperately want to fix it, but i just can't get him to stay still during nail trimmings now, and I'm concerned he'll try to bite because he's in pain :( he won't even stay still while sedated at the vet, i haven't tried it sedated at home yet. We have a grinder that's appropriate for his nail size. Due to his history, I'm concerned he'll bite. I don't want to have to pin him down or corner him. That'll just stress him out because he's already in pain. I read online just now that if you wait too long, you have to do it slowly to insure you don't hurt your dog. Am i past that point? Can I cut them now? They touch the tile when he walks, obvious, and they're worn down from exercise but not enough. Please, any responses are appreciated. I love my dog and I hate that I've left this gone on this long. I know I'm ignorant, but i no longer will be now that i know i caused him pain
