Cat spraying

My family’s cat has been spraying since we got him in 2016. He was 9 months old when we got him and he was already neutered. Sometimes the spraying happens once a month, sometimes several days in a row. We’ve been to the vet a couple of times and every time we’ve been told that he’s physically healthy. He has always been using his litter box.

In December 2020 he started taking anti-anxiety meds. They worked well, and a couple of weeks later my mum halved the dose from one pill to just half. He had an 8 month streak of no spraying, and four weeks ago or so he started doing it again on an almost daily basis, occasionally several times a day. Two weeks into the new “spraying phase” my mum upped the dose to one pill (as prescribed). The spraying is still going on and I can’t wrap my head around it. I have noticed that the spraying usually takes place at night, but while we’re still up.

There haven’t been any major or minor changes that I can think of. No one has moved in or moved out, there is no new furniture and he’s the only pet we have. We use the same brand and type of snacks, cat food and litter as we always have. He does not get any sort of punishment for spraying, and he’s never yelled at.

My mum is at her wits’ end and is considering taking him to an animal shelter. While I understand her I don’t want him to spend the rest of his life in an animal shelter. As a last resort, I’m willing to have him move in with me. I love him so much and he’s a very special cat that doesn’t get affectionate with anyone but me and my sister (and on some occasions my mum).

Has anyone had the same issue under similar conditions? Any and all advice is highly appreciated.
