We need help with our senior cat! (Peeing outside box)

My boyfriend and I recently adopted a senior shelter cat (her name is Koi) as an ESA for him. Koi is nearly 15, has had a bit of a hard life, and a complicated medical history. She currently has a managed thyroid condition (her last blood work was within the last month) and has a history of urinary tract infections, primarily caused in her life by neglectful care prior to our adoption. This factor has been ruled out for the present issue after a visit to the vet.

She has two boxes, one with a lid and one without (we haven't noticed a distinct preference) and she does use both of them regularly. They are cleaned at least once a day with the litter changed regularly. I have noticed some people on this subreddit have mentioned that their animals prefer their boxes not all the way clean, if anyone has experience with that I'd love to hear their take.

We feel like we're hitting all the major checkboxes for stressors for her in the environment. Just this morning (which led to me writing this) we found a pee-spot on the foot of our bed and I was prompted to reach out to you all asking for some help.
