Cat scratches head until she bleeds

My cat Paulette (2y) has been living with me and my husband since early January. We adopted her from the SPCA shortly after she had been spayed and treated for a yeast infection in her right ear. She had scratched at her ear until there was a lesion near the inside. We were given Animax ointment to treat the lesion and eventually it was healed. So we took the cone off of her- and she scratched a new cut in her head. So the cycle continued until I brought her to the vet. They looked into her ears to see if there were any issues and they found fluid inside that ear. I was given Tresaderm drops to treat the irritation. And Advantage to treat the possibility of ear mites or fleas. After that failed, she had scratched a spot near that ear completely bald! New lesions appear every time I remove her cone. I brought her back to the vet to see what the problem was- they didn't even know. They gave her a cortisone injection. Still scratching until she bleeds. They gave her an injection of antibiotics. Still scratching until she bleeds. I've changed her food, changed her litter, kept the house clean, called the SPCA to see if she had any other conditions- all to no avail. I've noticed that her skin is dry on her paw pads. And where her belly was shaved for her spaying, her nips are scabby looking. I feel so terrible about having to keep her cone on her, she tries to wiggle out of it all the time. Other than hating the cone, her disposition hasn't changed. She's very playful and cuddly even if she's suffering from dry, itchy skin. Does anyone have any idea what the problem could be??
