Help for my family cat with PTSD

My family cat (Sake) has ptsd. He just loves and adores my other two family cats. Sake is a bit silly, and has a total heart of gold. The thing about it is that my family doesn’t seek ways of taking care of the cats besides me. My mom feeds the cats and cleans the litterbox, while I groom play wash and clean for the cats. I recently started noticing that Sake has become overweight and is so scared of hands he doesn’t want to play even at long distances.

Sake was raised in a massive hoarding situation, and was shifted between 6 shelters and 2 homes during his upbringing in the first year of his life. Sake has been in our house for about 6 years now, and is very affectionate. Though I think I’m the only one who realized that he does nothing besides sleep in his safe space and eat all the food in the free feeder (plus the full can of wet food my mom feeds him for dinner). He still runs from everyone unless they have food, and refuses to play unless he’s lying down. This is a real big problem I need to solve, though I haven’t found any helpful advice. Any suggestions?
