Cat got an infection. Should I take him to the vet?

My cat got into a fight and now has an infection on the wound on his ear. When he got the wound at first we didn't know so we just washed the blood off with warm water like we usually do. Then a couple days later it seemed he was healing, but I noticed he was stinky. I didn't make the connection with the wound at first and my parents had been putting fertilizer in our garden, so we assumed he got curious and laid in it and that's where the smell came from. But then after a few days he was still stinky which is not normal because it should have worn off. So I started smelling around to see where it was coming from, and turns out it comes from his wound. We put multisporin on it (topical antibiotic) and we're keeping him in check, but I don't know if we're too late to put topical antibiotic. I'm worried it might have gone to his blood by now. What should I do?
