Chronic Ear Infection in Lab

My 13 year old Dudley Lab gets chronic ear infections. They are mostly yeast based, but some have been other (mites). He suffers from allergies and his skin has spots that are red that he licks constantly.

At 13, you would think we have it figured out. We changed him to a salmon-based food several years ago and it greatly cut back on our monthly vet trips, but in the last two years, they have been more and more frequent. He is often prescribed 2 weeks of Cephalexen, Mometamax drops, and ear wash. He has had two different allergy shots, neither of which worked. They don't really have a good preventative solution but I know he is miserable (and constantly rubbing his ear on stuff.)

What should I ask for at the vet to help tame his allergies and ear infections?
