Taking cat to the vet for the first time--do I need any documentation?

I'm taking cat to the vet for the first time--do I need any sort documentation? It's a relative's cat and he's away on a business trip--I don't have any paper work or history for the cat--all I know is his name and it doesn't have a collar (I just have a carrier).

I'm almost certain the cat is infected with ear mites as I've tried the olive oil treatment for 2 weeks and there is zero improvement. Will they refuse me without documentation or failure to provide any sort of information on the cat other than its name? What other costs can I expect besides the ear mite service? I assume they will prescribe me some Revolution--do I need to bring the cat for follow up visits or only if conditions doesn't improve?

Not really sure what to expect at all. The cat is known to poop on any car ride so I would like to be fully educated on the process and what I can expect before making an appointment.

P.S. Someone recommended me to this vet with very affordable prices ($10 for ear mites treatment); however, on their online pharmacy page, they only list 2 medications for treatment of ear infection. A quick search shows those prescription meds don't kill ear mites--only treat the symptoms. I assume I should avoid this vet as the pricing and products seem to reflect it's a one-time basic ear cleaning and I might need a more thorough approach?
