I need a better solution for my cats constipation/UTI

I adopted a 2 cats 2.5 years ago. They're sisters and are around 7 years old. One, Phoebe, is a normal little black cat, her sister Bella, was a 23 lb fat cat. the first week i had them Bella didn't poop at all and i learned the SPCA had her on a daily dose of miralax, and my vet said i could keep her on that, as it does work. It works, but there are issues, still 2.5 years later. She is down to about 16 lbs, but she still cannot clean herself. I have to wipe her butt after she goes to the bathroom, because the miralax sometimes makes her messy, and thats the problem. I'll get her dosage right, and it will be fine for a while, and then out of nowhere, she will either get constipated, or get diarrhea. The dose she needs is kind of always changing, and when she gets diarrhea, it ups her chances of getting a UTI (because she still cant clean herself), i do my best to clean her, but she's had about 5 or 6 UTI's since i got her and i'm really hoping i can find a non-diarrhea causing solution. She's only been eating wet food, as it helps her lose weight and is easier to digest. Ive tried pumpkin, and it doesn't seem to make much difference, does anyone have any experience? maybe some kind of food thats easy to digest where she wont need laxative? She does not have mega colon, we recently did an x-ray, but my vet doesn't seem to have any new approaches to deal with the problem, except some kind of surgery to remove excess skin around her vulva, to help prevent the UTI's, but that seems extreme and she's not good with anesthesia. She's a really good sport about everything, will eat anything i give her, and doesn't give me a hard time when she gets messy and needs a bath, i hope someone has some ideas, thanks
