My cat turned aggressive to my other cat after being neutered and hearing a trigger noise outside. It's been a month and I've tried everything I could think of. Please help!

I've had my two cats (Nemo -male and Bubbles - female) for four years.

I neutered Bubbles a month ago and since then she started to be aggressive towards Nemo, to the point where I had to keep them separate for a while as there was a bit of blood and screaming from both of them. Let me summarize some points:

  • Bubbles came from a shelter, she was on the street for 6 months
  • Bubbles is very sensitive to noises, if I have my phone on speaker during a call she will get so bothered that she will start attacking Nemo or come to paw my leg. She is growling when she hears children screaming outside or when there are cars making loud noises outside.
  • The first attack to Nemo started when she heard children screaming outside.
  • I separated them for two days, put them together only at feeding time, giving them treats together, played with both of them then slowly started to introduce them. Since then there was no more fighting with screaming but she is just attacking him all the time.
  • I plugged in Feliway and put no stress valerian drops on both of them at the advice of the vet
  • When Bubbles is sleepy, she is sometimes looking for Nemo and goes to cuddle with him and starts licking his head, Nemo does the same to her even when just walking past her so they obviously still love each other
  • However, when she sees him entering the room, she storms towards him and sometimes attacks him with her claws and even though he is hissing at her she is not pulling back, so I usually take her away
  • Nemo is now most of the times scared of her, he is walking very slowly around her, with his head down, obviously stressed that she will jump at him again and he is afraid to enter the room.
  • Sometimes after she runs to him she does nothing, depending on how he reacts. If he shows fear, she would start fighting him, if he doesn't, she just stops and lies down next to him.
  • Most of the times after she runs to him, they would just be sitting next to each other, in fight mode, moving their tails, avoiding eye contact and not move for the next half an hour. Or until one of them moves away and then the chase is back on.

Side notes:

  • Bubbles's neutering was done from the side of her body and she had to wear the suit to protect her from licking the wound. All the time with the suit on, she would pull out her fur from the back where the suit was closing. I had to remove it to make her stop.
  • Since the neutering she has a lump on her belly next to a nipple. that I already checked with the vet a week ago, they told me it's related to hormones, gave her a shot and told me it will be ok in a week. It's not yet back to normal so I am going to have her checked again as this worries me. She's also been pulling her fur from the lump.

What can I do to bring our life back to normal? They used to play together, sleep together for 4 years. Now for a month it's mostly attack chases. My flat is only two rooms in total so I can't keep them closed as they are always scratching the doors smelling each other and that's not a good life for any of us. Of course I am taking Bubbles for a blood test and lump check today.
