cat pooping pellets around the house for a year now. help?

So this has been going on for almost a year. Ever since my old vet recommended that I put my youngest cat (2yrs old) on diet food to help her lose weight (she was about 12 lbs at the time! she didn't even look big to us she just had a bit of a belly!) she has been pooping on/off around the house for a year now. They're pellet sizes, sometimes soft, other times more solid. Sometimes even liquid. I've tried helping her go back on her original food but to no avail. She was on the " Hill's Prescription Diet r / d Weight Reduction Chicken Flavor Dry Cat Food" and that's when everything started.

It took many attempts at which food would be suitable for her. She likes the Merrick Purrfect Bistro Grain-Free Chicken Pate Canned Cat Food a lot which I'm grateful for. Other kibbles I've tried to get her on was geared towards sensitive stomachs and gut health, despite her still pooping. Right now, my new vet recommended Royal Canin Veterinary Diet Hydrolyzed Protein HP Dry Cat Food which I guess is fine? Idk she still poops outside her little box sometimes so I can't tell if it's cause she's constipated or diarrhea or something?? We've had her x-rayed and we've had her poop looked at. She did find bacteria that were in her stool but says it could've been passed from her mother? Toxoplasmosis I believe it's called? (I'm not 100% sure I need to double check!!) If it was dormant and now it's just causing issues, which she then prescribed BiloVet Tylosin Tartrate Soluble Powder, which is weird cause:

1.) it's for chickens, turkeys, swine and honey bees ???? but I guess cats and dogs too???2.) my cat fucking hates it. she's picky as it is and has a sensitive nose so she smells it in her food3.) i have to force her to swallow the powder dissolved in water and i hate forcing her to do that

and i have no idea if it works or not ... like, it's not a cure-all. I know that much. I don't want to have to keep doing this to her forever. And sometimes when I find her pellets, there's blood and I'm really worried that her pooping hurts her so much that she's bleeding in her rectum.

I've had to have her clipped for sanitary reasons cause her stool if it's liquidy, gets caught on her back fur so now she's a little more buzzed on her bottom. It's easier to clean her, but I just feel so rotten cause I have to keep cleaning her and I know she hates it. She's used to it now but still, I know that's not normal and idk what to do. I wish I could get her an ultrasound or something else cause everything else the vet's prescribed hasn't given me any comfort to how she's doing overall. Has anyone dealt with this before? Cat's pooping outside litter box situation or something similar?

Any recommendations either would be nice. :(
