Hello! I want to get my dog insurance and I am about to enroll him to healthy paws insurance and its asking me if hes visited the Vet within 12 months. I got him from the shelter on 9/12/19 and received his medical history. It says evaluated 4/21/2018 but theres a date on the top right that says Rev Date 9/15/2019. I was wondering if that counted as a visit to the vet? Thanks for the help. Im unsure was Rev date means.
