Moving pet to EU (from non-EU country)

Hello, I'm moving to Paris next week with my cat.

For that I've done rabies vaccination, microchip and will get a health certificate.

I've brought this form to my vet and he and I read this form so hard, trying to understand all the fields 😂

I have two questions so far.

1. What do "Central competent authority" and "Local competent authority" mean?
2. What does it mean fields with diagonal? like I.2, I.6, I.8, I.9, ...
