Question regarding cat's bedtime behavior

I'll try to keep this brief. My cat is around 14 years old and he likes to spend time sleeping on my chest right when I go to bed. Lately however, he's been doing something a bit different. He will paw at my hand and pull it under his belly (he doesn't normally care for belly rubs) and begin kneading. If I move, he WAILS. He never makes the awful noise he makes if I move a muscle when he's doing this any other time. Sometimes, he will also straddle my arm and lock on. If I move or try to pet him, he cries loudly. He's not been doing this too long and I'm not sure what he's trying to tell me, if anything. I've also noticed when he's in this position, he seems very anxious. His tail will spasm at random. The vet told me he may be vision impaired so I'm not sure if this has something to do with him being afraid of the dark, but he seems to get around just fine at night (plus I keep a night light for him). Any ideas or experiences like this?
