Cat urinating - urinating directly outside litter box?

Hello! So I'm trying to decide if I should take one of my cats to the shelter. I would really REALLY like to not do this. However - we got these 3 cats when we made significantly more money. Since then we've fallen on hard times. We can afford the cats necessities and we can afford an occasional vet bill but vet visits have to be planned months in advance in order to save up. For the most part their health is manageable but one of our cats is peeing directly outside the litter box and I can't for the life of me figure out why!

We have 3 litter boxes - of which the cats tend to use 1 the most. That's the one she pees directly outside of. She doesn't poop outside the box and I have seen her urinate in the box but there's no pattern to it? Like I've seen her pee outside the box directly after I've cleaned it - so it's not that its too dirty. I've tried moving the box around but she'll just move where she pees to directly outside it again. I've tried a bigger litter box. I've tried different types of litter. I've cleaned the floorboards with enzyme cleaners. Nothing makes a difference! Right now I've resorted to placing puppy pads around the litter box so the floor doesn't get destroyed and my landlord doesn't make me pay for it.

We took her to the vet a few months ago and there are no health issues. She gets along with the other cats well. The only thing that I can think of is that my apartment building is really shitty and despite them being indoor cats we have an ongoing problem with fleas. The cat who is peeing is the one most affected by the fleas (not sure why?). We have an ongoing cycle where we clean the apartment top to bottom, quarantine the cat, and use flea collars etc. Usually the fleas are eliminated for a few weeks and then somehow make it back in. I haven't been able to afford the very expensive flea treatments - and I'm thinking of saving up to try them. But we have 3 cats and I feel that in order to fully get rid of the fleas I'd need a treatment for all of them - extra expensive.

Please let me know if you have ANY ideas on what to do. I want to take her to the shelter as a last resort but I can't continue to have her pee outside the box. I can plan to take her to the vet again but it won't be for at least 3 more months, so if there is anything I can do in the meantime I would like to try it.
