Cat viciously scratching ears

Hey. My 16-year-old female cat has had an issue for the past several years with scratching her ears viciously. We've taken her to a few different vets, gotten different antibiotics, and allergy shots, but absolutely nothing has stopped this problem. We've been doing what the vet says to do, put antibiotics on, give her a collar, wait until it's healed, and then take it off. Nothing is getting rid of this issue. Anytime you scratch around her ear area, her back leg starts twitching and will eventually start scratching just out of habit. She also has issues rubbing her eyes until the area around them is all scratched up and bloody. Is there something we should do? There's not fleas or small creatures, at least according to the vet, and antibiotics aren't helping. She's gotten an allergy shot several times and she still scratched with it. It's not food allergies, we've tried every food-type out there, and the scratching persists. Does anyone have any suggestions? She had a collar on, but vomited today. We took it off to clean it and in that time she ripped her ears and face again. What should we do???
