Euthanasia Options for Cat

I have felt that I need to let go of my twelve year old cat for a couple years. He was diagnosed diabetic and we gave him insulin and got him special food that was good for diabetic cats. It helped for a while but he's not using the litterbox and is having diarrhea frequently. We've spent a lot on vet visits and we can't afford to keep paying for the vet for testing and then increases in insulin if necessary, more testing, etc.

Additionally, he caused some damage to our previous house from peeing. We paid our landlord for damages but he told our current landlord and now our current landlord is being patient, he knows it's hard to let go of a pet, but he's understandably wary.

I am considering surrendering my cat to the humane society but I am certain, with his health issues, he will be euthanized and I would feel horrible if I weren't with him as he passes so I'm also considering just requesting he be euthanized at the vet. We just moved to a new state, haven't established with a vet, so I'm not even sure if they would euthanize without seeing him first. I just don't know. But part of me feels guilt, questioning if he's really so unhealthy that euthanasia is the right choice. Am I letting him go more for my convenience than for his health? And there's also something that feels like greed involved. He was my first pet of my own. He has traveled with me to college out of state, to grad school, to my first home with my wife, and now to our second home. I feel like letting go of him would be like letting go of one of the only constants in my life but that feels like greed.

I don't know what to do and welcome any advice.
