Relocating dog from Hong Kong to USA questions

Hey everyone,

Ive got a few questions about moving my dog to the States that I'm hoping some of you can help with. First, I have reviewed websites for the CDC, CBP, airline regulations, and state regulations. However they're not particularly clear for people trying to do this on their own.

The dog is a 3 year old golden retriever. Up to date with vaccines (no rabies though, but HK isn't on the rabies list), chipped, and in good health. We'll be flying direct from HK to Boston via Cathay Pacific. Specific questions are as follows:

  1. We called our vet to schedule an appointment and they said they need a form stating the regulations for import to the US. After scouring the web, I haven't been able to find a form specifically for importing dogs. Is it possible all we need is a written letter from our vet stating our dog has a clean bill of health?
  2. The dept of Agriculture requires dogs from "at risk" countries to have a rabies vaccine. China is on that list but there's no mention of Hong Kong. Thats not something we're required to have, correct?
  3. On the dept of Agriculture website, Hong Kong is on the list for African Swine fever and Highly Pathogenic Avian Influenza. Are these something my vet needs to sign off on or include in the letter?
  4. As of right now, is there any possibility of entry denial or possible quarantine due to coronavirus? As far as I can tell there have not been any new regulations but I haven't been able to get in touch with the CDC yet either.

Any other advice would be much appreciated. Also, we did contact going through an agency but were quoted 5k-7k so we'll be handling this ourselves. Thanks guys, getting nervous about this with the lack of information available online.
