Senior dog, mind is there but her body isn’t. Advice?

I have a 12 year old newfie/great pryenees mix dog. She has always had trouble with her hind legs because she is large and had a double ACL surgery back in 2016. She recovered amazingly and lived beyond our expectation. However, this week she has significantly been acting different. It’s mainly just that she can’t get up or walk without help/falling, and that she pees or poops herself. She has been eating just the same. Her breathing is a bit labored. You can tell her mind is fully there but she physically can’t do much. My parents believed she was going to pass overnight peacefully which is why we haven’t taken her to a vet yet. We know she is old and she is pretty big so we didnt want to tire her out taking an unnecessary vet trip. What I’m asking is- what do you do when your old dogs body isnt there but shes totally competent and her mind is fully there?
