Tips for flying with a high-stress cat?

My SO and I want to take our high-stress cat with when we move to the Netherlands in August (from California), as we don't think anyone would be willing to adopt him (he's only nice to us).

Unfortunately, we are very concerned about his health and safety. He can't go to the vet without sedation and typically urinates in his crate from stress. I can't imagine him on a 10-hour flight in the hold (there's no way we could get him through security to be in the cabin... no way it would work to take him out of his crate for security).

Does anyone have any tips for how to manage this situation? We are really stuck between a rock and a hard place.
