Cat peeing on carpet sporadically - out of options

We have a 5 year old male adopted 6 months ago. He's neutered and UTD on all shots. We've had no issues with him since the adoption, but he's suddenly urinating on the carpet. We caught him 2-3 times in the office and 2-3 times in the bedroom, usually in the same spots. Vet has checked his urine and he's healthy, we've switched litter boxes and litter, even adding a fourth litter box in a small apartment with two cats. He goes just fine in the litter boxes, but might pee on the carpet like 40% of the time.

Not only does our carpet stink (we've tried Resolve and ACV/water, waiting on an Amazon order of more deodorizers), but he hasn't stopped trying to pee. We've limited access to the bedrooms (carpeted), but he manages to wander in unnoticed sometimes and tries to pee.

Basically, what else am I missing? No other stressor has occured for him to be acting out. Our other cat is an 11 year old male, neutered, who has had no issues going outside the box since I've had him so I have zero experience with this.
